 I used to think osteoporosis was something we would notice as our bones became weaker. But what concerned me most as I learned more about it is that bone loss can progress quietly, and sometimes a fracture is the first clear warning sign.

1. What Is Osteoporosis and Why Does It Develop?

We often think of bones as solid structures that stay essentially the same throughout adulthood.

They do not.

Bone is living tissue that is constantly being renewed. Old bone is broken down and new bone is formed throughout life.

During childhood and young adulthood, the body builds bone rapidly. Later in life, the balance gradually shifts. When bone is lost faster than it is replaced, bones can become less dense and weaker.

Osteoporosis is a disease in which bones become weaker and more likely to fracture.

The word itself is sometimes translated as “porous bone,” but the important issue is not simply that the bone looks less dense.

A healthy bone has a strong internal structure. With osteoporosis, the amount and quality of bone decline, making the skeleton less able to withstand stress.

This can make a fracture more likely from an injury that might not have broken a stronger bone.

Imagine someone in her late 60s who feels healthy, walks every day, and has never thought much about her bones.

One morning she slips while stepping off a curb and breaks her wrist.

During the follow-up, her healthcare professional asks whether she has ever had a bone density test.

She has not.

Further evaluation shows that her bones are much weaker than she realized.

This kind of situation explains why osteoporosis is sometimes described as a silent disease.

You cannot reliably tell how strong your bones are by how you feel.

Where Do Osteoporosis-Related Fractures Commonly Occur?

Osteoporosis can affect bones throughout the body, but fractures are particularly concerning in areas such as the:

  • Hip

  • Spine

  • Wrist

A vertebral fracture in the spine may sometimes occur with surprisingly little trauma.

Some spinal fractures may cause back pain, while others may initially go unnoticed. Multiple vertebral fractures can eventually contribute to height loss or changes in posture.

Hip fractures are particularly serious because they can have a major effect on mobility and independence.

Why a Fracture Matters More as We Age

A broken bone at age 25 and a broken bone at age 80 may have very different consequences.

But they are very different.

A person can have osteoporosis, osteoarthritis, both conditions, or neither.

Joint pain alone does not tell you whether your bones are weak.

What Is Osteopenia?

You may also hear the term osteopenia, often called low bone mass.

Osteopenia means bone density is lower than what is considered normal for a healthy young adult, but it has not reached the bone-density threshold used to define osteoporosis.

That does not mean it should be ignored.

Fracture risk depends on more than a single bone-density number. Age, previous fractures, medications, family history, smoking, body weight, and other factors also matter.

We will look at DXA scans, T-scores, osteopenia, and osteoporosis much more closely in Section 3.

Aging and Bone Loss

Some bone loss occurs naturally with aging.

But osteoporosis is not something every older adult inevitably develops.

The amount of bone you built earlier in life, how quickly you lose it later, your hormones, nutrition, physical activity, genetics, medications, medical conditions, smoking, alcohol use, and other factors can all influence bone health.

For women, one particularly important transition is menopause.

Why Menopause Matters for Bone Health

Estrogen plays an important role in maintaining bone.

During perimenopause and menopause, estrogen levels change and eventually become much lower.

Bone loss can accelerate around and after menopause, which is one reason osteoporosis becomes increasingly important for women in midlife and later life.

That is why simply taking a calcium supplement cannot guarantee that osteoporosis will be prevented.

You cannot change your genes, but knowing your family history can help you and your healthcare professional decide whether earlier risk assessment or bone-density testing may be appropriate.

Low Body Weight Can Increase Risk

People with lower body weight may have less bone mass and may be at greater risk of osteoporosis and fracture.

This does not mean that gaining excess weight is a strategy for protecting your bones.

Obesity carries its own health risks, and body weight is only one part of bone health.

Smoking Can Harm Bone Health

Other medications may also influence bone or fracture risk depending on the drug, dose, duration, and individual.

But there is an important rule:

Never stop a prescribed medication because you are worried about osteoporosis without talking with your healthcare professional first.

The medication may be essential for treating another condition.

Instead, ask whether your treatment affects bone health and whether you need additional monitoring or preventive measures.

Risk Factors Can Add Up

One risk factor does not automatically mean you have osteoporosis.

But several risk factors together may be more concerning.

That is why age alone should not be used to judge bone health.

Who Should Be Screened?

Current U.S. Preventive Services Task Force recommendations provide an important starting point.

Women age 65 and older:
Screening for osteoporosis is recommended.

Postmenopausal women younger than 65:
If one or more osteoporosis risk factors are present, clinical risk assessment can be used to determine whether the woman is at increased risk and should receive bone-density screening.

Men:
Men can absolutely develop osteoporosis, but the USPSTF currently concludes that evidence is insufficient to determine the overall benefits and harms of routine population screening in men.

That does not mean men should ignore bone health.

A man with a fragility fracture, long-term glucocorticoid use, or other significant risk factors may require evaluation based on his individual medical situation.

Screening Is Not the Same as Diagnosis

This distinction is important.

Screening means looking for osteoporosis in someone who does not already have a known diagnosis or a previous fragility fracture.

If you have already had a fracture from relatively minor trauma, have a condition known to cause secondary osteoporosis, or take medication strongly associated with bone loss, your healthcare professional may use a different evaluation pathway.

General screening recommendations are not a substitute for individualized medical care.

A Real-Life Question Worth Asking

Imagine a healthy-looking woman who has just turned 65.

She exercises, eats well, and has never broken a bone.

She thinks:

“I feel fine. Why would I need a bone-density test?”

That is exactly why screening exists.

Osteoporosis can be present without pain or obvious symptoms.

The purpose of screening is to identify people at increased fracture risk before the first serious fracture occurs.

Simple Bone-Risk Check

Ask yourself:

Am I 65 or older?

Have I gone through menopause?

Do I have low body weight or a parental history of hip fracture?

Do I smoke or drink alcohol excessively?

Have I used glucocorticoids or other bone-affecting medicines for a long time?

Do I have a medical condition that may affect my bones?

Have I ever had a fracture after a relatively minor fall or injury?

One “yes” does not diagnose osteoporosis.

But your answers can help start a useful conversation with your healthcare professional about bone health, fracture risk, and whether you need further evaluation.

3. How Is Osteoporosis Diagnosed? Bone Density Tests Explained

Osteoporosis usually does not announce itself with pain or obvious symptoms.

That creates an important question:

If you cannot feel your bones getting weaker, how do you know whether you have osteoporosis?

One of the most important tools is a bone mineral density test, commonly performed using dual-energy X-ray absorptiometry, or DXA. You may also see it written as DEXA.

The test can help identify low bone density before a serious fracture occurs and can provide information that healthcare professionals use when evaluating future fracture risk.

What Is a DXA Scan?

A DXA scan uses a very low dose of X-rays to measure bone mineral density.

The test commonly measures areas that are particularly important in osteoporosis, including the hip and lumbar spine.

A typical DXA scan is:

  • Noninvasive

  • Usually painless

  • Relatively quick

  • Performed while you lie on a padded table

  • Different from an MRI or CT scan

You do not go inside a long enclosed tunnel like you might during some MRI examinations.

A scanning arm passes over the body while the machine measures bone density.

For most people, the experience is much simpler than they expected.

What Does a DXA Test Actually Tell You?

The DXA scan estimates how much mineral is present in certain bones.

The result can then be compared with reference values.

For postmenopausal women and men age 50 and older, one of the measurements commonly used to interpret bone density is the T-score.

This is where many people become confused.

A T-score is not a percentage.

It compares your bone mineral density with the average bone density of a healthy young adult reference population.

Understanding Your T-Score

For postmenopausal women and men age 50 and older, the commonly used categories are:

This last point is important.

A T-score does not tell the whole story.

Osteopenia Does Not Mean “Almost Osteoporosis”

A woman receives a report showing a T-score of −1.7.

She reads the word osteopenia and immediately thinks:

“Does this mean osteoporosis is inevitable?”

No.

Osteopenia means that bone density is lower than the normal young-adult reference range but has not reached the DXA threshold used to define osteoporosis.

Some people with osteopenia will never progress to osteoporosis.

At the same time, fractures can occur in people whose T-scores are in the osteopenia range, particularly when other risk factors are present.

That is why healthcare professionals do not look only at whether your T-score crosses one particular line.

Bone Density Is Only One Part of Fracture Risk

Consider two women who both have a T-score of −1.9.

The first is 57, physically active, has never fractured a bone, does not smoke, and has no major additional risk factors.

The second is 78, has previously had a fracture after a minor fall, has a parent who fractured a hip, and takes medication that can weaken bone.

Their T-scores are identical.

Their overall fracture risks may not be.

Bone density matters, but age, previous fractures, medications, family history, smoking, alcohol use, fall risk, and other clinical factors matter too.

What Is FRAX?

Healthcare professionals may use fracture-risk assessment tools along with bone density and medical history.

One widely used tool is FRAX, which estimates the probability of certain major osteoporotic fractures and hip fracture over the next 10 years.

FRAX considers several factors that can include:

  • Age

  • Sex

  • Weight and height

  • Previous fracture

  • Parental hip fracture

  • Smoking

  • Glucocorticoid use

  • Certain secondary causes of osteoporosis

  • Alcohol use

  • Femoral-neck bone mineral density when available

FRAX is a risk-assessment tool, not a home diagnosis.

Its results need to be interpreted in the context of the individual and the clinical guidelines being used.

Who Should Get Osteoporosis Screening?

Current U.S. Preventive Services Task Force recommendations are especially important here.

Women age 65 and older should be screened for osteoporosis to help prevent osteoporotic fractures.

For postmenopausal women younger than 65, the approach is different.

Rather than sending every woman under 65 for a DXA scan automatically, clinicians first consider osteoporosis risk factors.

If risk factors are present, a clinical risk-assessment tool can help determine whether screening is appropriate.

This does not mean men cannot develop osteoporosis.

They can.

It means there is currently not enough evidence for the USPSTF to recommend routine screening of the general male population in the same way it does for women age 65 and older.

Men with significant risk factors, previous fractures, or certain medical conditions may still need individualized evaluation.

Screening Recommendations Do Not Apply to Every Situation

There is another important distinction.

General screening recommendations are designed for people who have not already been diagnosed with osteoporosis and do not have certain conditions that clearly require evaluation.

Someone who has already had a fragility fracture, has a medical condition strongly associated with bone loss, or uses certain bone-affecting medications may need evaluation regardless of the routine screening pathway.

This is why an online age cutoff should never replace an individualized medical assessment.

What Is a Fragility Fracture?

You may hear healthcare professionals use the term fragility fracture.

This generally refers to a fracture occurring from relatively low-level trauma—such as a fall from standing height or less—that would not normally be expected to break healthy bone.

A fracture like this can be an important warning sign.

If an older adult breaks a hip, wrist, spine, or another bone after a relatively minor fall, it is reasonable to ask:

“Could weak bones have contributed to this fracture?”

That question can lead to an osteoporosis evaluation that might otherwise have been missed.

“My DXA Was Normal Five Years Ago. Am I Done?”

Not necessarily.

Bone density changes over time, but there is no single repeat-testing interval that is appropriate for everyone.

How soon another DXA scan may be useful depends on factors such as:

  • Your initial bone-density result

  • Age

  • Changes in medical conditions

  • New fractures

  • Medication use

  • Whether you are receiving osteoporosis treatment

  • Your overall fracture risk

For some people, repeating a test too soon may add little useful information.

For others, closer monitoring may be appropriate.

Your healthcare professional can determine whether and when another test is needed.

DXA Result: What Happens Next?

A bone-density report should lead to a conversation, not panic.

“Do I have osteoporosis—yes or no?”

A Real-Life Example

Imagine a 67-year-old woman having her first DXA scan.

She feels healthy and has never broken a bone.

Her report shows a T-score of −2.6 at one of the measured sites.

Her first reaction is fear:

“My bones are terrible. Am I going to break my hip?”

A T-score in the osteoporosis range is important, but it does not predict exactly what will happen to one individual.

Her healthcare professional looks at the entire picture: age, bone-density results, medical history, medications, fall risk, family history, and other factors.

Together they discuss nutrition, exercise, fall prevention, and whether osteoporosis medication is appropriate.

The diagnosis becomes information that can guide prevention and treatment, rather than a reason to assume a fracture is inevitable.

That relationship is why they are so often discussed together.

But there is also a common misunderstanding:

“If calcium and vitamin D are good for bones, taking more must be even better.”

That is not how nutrition works.

The goal is to get enough, not as much as possible.

Calcium: A Major Building Block of Bone

Almost all of the calcium in the body is stored in the bones and teeth.

Calcium helps give bones their structure and strength, but the body also needs calcium for functions such as muscle movement, nerve signaling, and blood vessel function.

If calcium intake is consistently inadequate, the body still needs calcium for these essential functions.

That is one reason adequate calcium intake is important throughout life.

How Much Calcium Do Adults Need?

The recommended amount depends on age and sex.

Notice that women age 51 to 70 have a higher recommended intake than men of the same age.

These amounts refer to total calcium intake from food, beverages, and supplements combined.

That last word matters.

If you already get a substantial amount of calcium from your diet, you should not automatically add another 1,200 mg in supplement form.

Start by Looking at Your Food

Before reaching for a supplement bottle, ask:

“How much calcium am I already getting from what I eat?”

For many people in the United States, milk, yogurt, and cheese are major sources of calcium.

But dairy foods are not the only sources.

Calcium can also be found in foods such as:

  • Milk, yogurt, and cheese

  • Canned sardines with edible bones

  • Canned salmon with bones

  • Kale

  • Broccoli

  • Bok choy

  • Calcium-set tofu

  • Calcium-fortified soy or almond beverages

  • Some fortified juices and breakfast cereals

If you use plant-based milk alternatives, check the Nutrition Facts label.

Calcium content can vary considerably between products.

Food First: A Simple Calcium Pattern

A bone-friendly day does not have to look like a shelf full of supplements.

It might look something like this:

The point is that calcium can be spread throughout the day rather than treated as one large dose from a bottle.

What If You Don’t Eat Dairy?

You can still get calcium without drinking cow’s milk.

People who are lactose intolerant, vegan, or simply do not like dairy products can choose other calcium-containing foods.

Fortified plant beverages can be particularly useful, but read the label.

A beverage that looks like milk does not necessarily contain the same amount of calcium.

Some people may also need to pay closer attention to overall protein, vitamin D, and other nutrients when eliminating entire food groups.

Why Vitamin D Matters

Calcium cannot do its job alone.

Vitamin D helps your body absorb calcium.

It also plays important roles in bone and muscle health.

Unlike calcium, vitamin D naturally occurs in relatively few foods.

That makes “just get some sunshine” an unreliable prescription for everyone.
